Eye of Cthulhu Mask is not something you find lying around — you craft it, and the recipe is below.
The fight is at the back of the Sealed Realm of Terraria, and the map carries exactly one entrance pin for that realm — southeast of Fisherman's Point, with its own fast travel statue at the door. Unlock the statue on your first trip and every repeat run starts a few steps from the boss.
Using Eye of Cthulhu Mask Well
Do not farm the 10% roll. The craft costs a single Hallowed Bar and one guaranteed kill pays out 20–30 of them, so the first clear you finish already covers the mask plus a long stack of spares — the drop is a bonus, never the plan.
Eye of Cthulhu is Dark-element, which means Dragon Pals hurt it most and Neutral Pals are the worst thing you can bring, since Dark is the element that beats Neutral. Sort that before the first attempt rather than after a wipe.
Eye of Cthulhu drops the mask itself at a 10% rate, one per kill, on top of the 20–30 Hallowed Bar it hands over every single time. So roughly one clear in ten ends with the finished helmet in your bag without you crafting anything.
