The Medicine Rack is the level 24 Infrastructure shelf that lets sick base Pals walk over and dose themselves instead of waiting on you to open their menu one at a time. This guide covers the technology cost, the three build materials and the raw pile underneath them, which Medical Supplies cure which ailment, and the Pals worth parking on a medicine bench to keep the shelf full.

Medicine Rack at a Glance

This is a passive shelf, not a workstation — it has no worker slot, so it never eats one of your base Pals' assignments the way the Clinic on the same technology row does. That makes it the cheapest health fix in the game to run: build it once, drop medicine inside, and it works for free forever. The only real cost is the 40 Ore that turns into its 20 Ingot, which is why the Ore Mining Site unlocking on the exact same row is such convenient timing.

How to Unlock the Medicine Rack

Hit character level 24, open the Palworld technology tree, and spend 2 points. That part is trivial — the problem is that level 24 is one of the busiest rows in the tree, with nine technologies asking for 19 points in total, and most players arrive with far fewer banked. If you have to prioritise, the Medicine Rack and the Clinic are the two that pay you back every single in-game day, while the three saddles only matter for the specific Pal you actually ride. Check how to get more technology points if the row leaves you short.

Build Materials and Where to Make Them

Three materials, two stations, nothing exotic. The 20 Ingot is the whole bill in practice — 40 Ore goes into the furnace for it, plus another two for the nails inside the boards. The Cloth and Wooden Board both come off a Primitive Workbench in a couple of minutes. Add it all up and the rack costs 42 Ore, 38 Wood and 10 Wool, or roughly 6,650 gold of raw material at merchant prices, which is nothing for a permanent base fixture.

Fastest Sources for the Raw Materials

Ore is the only line that can actually stall you, and there is no shortage of it on the map — 1,632 Ore nodes are scattered across the Palpagos Islands, so the real question is whether one sits inside your base radius. If it does, the Ore Mining Site from this same technology row turns those nodes into a hands-off supply. Wool is easier: a Ranch with a couple of sheep Pals covers the 10 you need overnight, and Lamball is both the most common of them and the one you probably caught in the first hour.

What the Medicine Rack Does

Base Pals pick up ailments while they work — they get sick, sprain something, catch an ulcer, break a bone, or slide into Weakened and Depressed as their SAN falls. Normally you notice the little icon hours later and hand-feed the cure. With a Medicine Rack placed and stocked, an afflicted Pal walks to the shelf and treats itself, which means a colony of twenty workers no longer needs you playing nurse. The catch is in the name: it is a shelf, not a factory. It dispenses what you put in it and nothing more, so an empty rack is decoration.

Where you place it matters more than people expect. Pals path to it from wherever they are working, so a rack buried behind three walls at the far corner of a big base costs each sick Pal a long round trip. Drop it near the middle of your work floor, ideally beside the Clinic and the Pal beds, so the whole health cluster sits in one place.

TipPrevention beats treatment. The Clinic on this same row reduces SAN depletion and cuts down on illness in the first place, and a hot spring plus a decent bed keeps SAN topped up — see the best food for SAN and hot springs. Use the rack as the safety net, not as the plan.

Which Medicine to Stock

Three tiers of Medical Supplies exist, and they are not upgrades of each other — each one cures a different set of ailments, so a rack holding only the expensive tier still leaves a Pal with a sprain untreated. Keep a stack of all three inside. None of them restore HP, which surprises people: the Recovery Meds line does the healing, the Medical Supplies line does the curing. Low Grade Medical Supplies is the one you burn through fastest and the cheapest to mass-produce at 5 Red Berries and 2 Horn a pop, while High Grade Medical Supplies is the one worth buying from merchants when you are short, since Weakened and Depressed hit exactly when a Pal is already dragging your base output down.

Low Grade Medical SuppliesSick, Sprain, Overfull5 Red Berries, 2 Horn780 Gold
Medical SuppliesUlcer, Fracture3 Ingot, 3 Horn, 1 Bone1,820 Gold
High Grade Medical SuppliesWeakened, Depressed5 Ingot, 5 Horn, 2 Bone3,120 Gold

All three are sold by the Village, Caravan, Desert, Volcano and Wander merchant families, so a shopping run is a legitimate substitute for crafting early on — the full merchant stock list shows who carries what. They also turn up in chests: Low Grade in the early caverns like Hillside Cavern and Ravine Grotto, Medical Supplies in mid-game spots such as Volcanic Cavern and Cherry Blossom Cave, and High Grade mostly out in Feybreak Cavern and the oil rig crates.

Horn and Bone Farming

Two of the three recipes lean on Horn and Bone, and both drop at a guaranteed 100% rate from the right Pals, so a single farming loop keeps the rack stocked indefinitely. Elgrove is the best Horn target of the wild spawns at 2-3 per kill, with Eikthyrdeer a flat 2 and far easier to find at low level. For Bone, Anubis gives the biggest haul at 3-5, though Cawgnito at 2-4 is the one you can farm at night without a fight you have to plan around. Both Bone Pals can also be parked on a Ranch to produce it passively.

Where the Medicine Comes From

The rack does not make medicine — a medicine workbench does, and there are three of them. The Medieval Medicine Workbench unlocks 12 levels before the rack for 30 Wood, 5 Nail and 10 Paldium Fragment, needs no electricity, and handles every Medical Supplies tier you actually need for ailments. That is the one to build alongside the rack. The later benches exist for the Recovery Meds and Revival Potion end of the catalogue — the full breakdown is in the Advanced Medicine Workbench guide. Worth noting separately: the Witch Cauldron at Lv. 32 speeds up medicine manufacturing across your base, and building a second one does nothing, so place exactly one.

Best Pals to Brew the Refills

Medicine Production is one of the rarer work suitabilities — only 43 Pals have it at all, and just 11 of them reach Lv. 5 or better. Silvance is the only Pal in the game with Lv. 8, which makes it the outright best bench worker, and it doubles as a Lv. 6 Planting and Handiwork body when the bench queue is empty. If Silvance is out of reach, Wistella is the practical pick: Lv. 6 Medicine Production on a common Pal you can breed early, versus Lv. 7 on a raid boss like Bellanoir Libero that most players will not have for a long while. Dandilord is the value pick if you want one Pal covering the medicine bench and the plantation at once.

Slot whichever one you land on into the bench rather than leaving it to base wandering, and check the full Medicine Production Pal list for the other 35 options if none of these are in your box yet.

Build These Alongside It

The Medicine Rack is one piece of a small health cluster, and the rest of it is cheap. The Clinic is the big one — same level, same 2 points, and it attacks the problem upstream by reducing SAN depletion and preventing illness, at the cost of one Medicine Production worker. The Fluffy Pal Bed on this same row is where injured Pals actually rest, and hot springs handle SAN recovery from Lv. 9 onwards. Later on, the Pal Surgery Table at Lv. 38 is a different job entirely — it rewrites passives and gender rather than healing anything — and the Ancient Clinic at Lv. 73 is the endgame replacement for the basic one.

Summary

Short version: hit level 24, spend 2 technology points, and build the rack from 20 Ingot, 5 Cloth and 3 Wooden Board — 42 Ore, 38 Wood and 10 Wool at the raw end. Put a Medieval Medicine Workbench next to it with a decent Medicine Production Pal on the job, keep all three Medical Supplies tiers stocked inside, and your base stops needing manual nursing. Take the Clinic on the same row while you are there; between the two, base illness mostly stops being something you think about.
